Agape Allergy accepts most major health insurance plans. If you don’t see your carrier listed, please contact your plan’s member services to confirm that Agape Allergy is “in-network.”
Please note that coverage for allergy services—including testing and allergy shots—may vary. Be sure to review your plan’s eligibility, benefits, limitations, and exclusions to understand what is covered.
Patients are responsible for obtaining a referral from their primary care provider if required by their insurance.

Patient Responsibility for Referrals
Some insurance plans require a referral from your primary care provider before scheduling with a specialist. It is the patient’s responsibility to obtain and provide any required referrals prior to your appointment. Without a valid referral, your insurance may not cover the visit, and you may be responsible for the full cost of services.

- Android: Android 7.0 and higher
- Explanation as to the requirements: We believe that this is a necessary update to help protect your practice and patient information. Once Apple releases a new operating system, it no longer supports its older iOS versions, and it is not possible to ensure that unsupported devices and operating systems remain compliant with industry security standards.
Any telehealth appointment that needs to be cancelled due to incomplete or late check-in resulting in the appointment not being able to commence could result in a $75 late cancellation fee.
