Self-Scheduling Policy for Patients
To ensure smooth scheduling and insurance verification, please review the following important guidelines:
For Patients with HMO Insurance Plans:
If you are self-scheduling an appointment and have an HMO plan, you must obtain a referral from your Primary Care Provider (PCP) before booking your visit.
- If a referral is not received prior to scheduling, your appointment will be canceled until the referral is on file.
- This helps ensure your visit is covered and avoids unexpected charges.
For All Self-Scheduling Patients:
To confirm your appointment and verify that we are in-network with your insurance provider:
- Please log into your AGAPE PATIENT PORTAL on this website.
- Upload a clear copy of the front and back of your insurance card as an attachment to a portal email message to Agape Allergy Scheduling Staff.
- If we do not receive your insurance information within 72 business hours, your appointment will be canceled until the required documentation is submitted.
Referrals: For your convenience, please email a copy your insurance referral to info@agapeallergy.com at least 72 business hours ahead of your appointment.
