ACCEPTED INSURANCES

Below is our list of accepted insurance plans.

Patients are responsible for obtaining a referral if required by their insurance.

Please note: We are not accepting new MassHealth, CCA, or WellSense patients; existing patients remain covered.

Insurance that REQUIRES Referral

  • Aetna (HMO, EPO Elect Choice, Select)
  • Blue Cross Blue Shield (HMO, Network Blue, Medicare HMO)
  • Cigna (HMO, Network POS)
  • Harvard Pilgrim (HMO, HMO POS)
  • Tufts (HMO, EPO, POS, Select, Direct ConnectorCare II & III [Subsidized], Direct Platinum [Non-Subsidized])
  • UnitedHealthcare (ID Card/Payor site will state if referral is needed)
  • MassHealth Standard PCC Plan (Existing patients only)

Insurance that DOES NOT Require Referral

  • Aetna (PPO, Open Access, POS Choice II)
  • Blue Cross Blue Shield (PPO, Direct, EPO, FEP)
  • Cigna (PPO, Open Access, Indemnity)
  • Harvard Pilgrim (PPO, Medicare Enhance, Open Access)
  • Health New England (Commercial Plans only*, Medicare & Supplement Plans)
  • United Medical Resources (UMR) (Commercial)
  • Unicare GIC / Wellpoint
  • UnitedHealthcare (Choice Plus, Senior Care Plan)
  • Blue Benefit Administration of Massachusetts [BCBS-MA] (PPO)
  • Anthem Blue Cross Blue Shield (HMO, PPO)

Payment Policy:

**Payment policy effective January 1, 2025.

Payment at the Time of Service: All payments, including copays and any outstanding
balances, are due at check-in. Coinsurance and deductible payments are due upon
completion of your appointment.

  • Missed Payments: Failure to pay your financial responsibility at the time of service may
    result in your appointment being rescheduled.
  • Accepted Payment Methods: We accept Visa, Mastercard, American Express, Care
    Credit, and cash.
  • Outstanding Balances: Patients with unpaid balances after receiving three statements
    with no effort to pay will be referred to a collection agency, which may result in dismissal
    from the practice.

If there is any discrepancy between what was collected and what is owed, we will promptly
issue a refund.

For any questions regarding your deductible, copay, or explanation of benefits, please contact
your insurance provider directly.

Financial Assistance:
We understand that financial challenges can arise. If you are experiencing hardship, please speak with a member of our team or the office administrator. We will do our best to accommodate you with a payment plan.

Private Pay:  We offer a discount for private pay patients as well as payment plans. Our billing staff is available to answer your billing and insurance questions at 413-707-7720.

Allergen immunotherapy:  Allergy shots are your customized prescription based on your testing results. If you decide after you have signed your consent and confirmed your appointment that you no longer wish to get your shots, we must still bill you for the vials. We understand that life happens and we will keep your vials for you until the expiration date.
